Your information. Clear boundaries.
CareWella is an initiative of Chusommy Ventures Limited, RC 1614621. CareWella does not replace your healthcare provider’s clinical record system or independently certify clinical services.
Private accounts and fictional demonstrations
My Care contains your saved requests and records shared with your account by administrators. Vaccination entries, laboratory results, referrals, follow-up information and in-app reminders persist when saved. Only released care updates are visible to the intended patient; drafts and all saved updates are accessible to app administrators. The separate Explore Demo area uses invented profiles and memory-only state, including fictional vaccination names and administration dates. It does not populate your personal account, and refreshing or resetting the demo clears the example entries.
Current pilot-preparation safeguards
CareWella is operated by CHUSOMMY VENTURES LIMITED. New patient identity, diary, appointment, screening, vaccination, referral and clinical-update writes are paused by a protected clinical-release gate. Account registration and participation review never enable this gate or grant administrator privileges. Any existing records remain subject to their owner, intended-recipient and administrator access rules. Only workspace editors can change the release gate after documented independent security, legal and clinical approvals; app account setup cannot do so. A platform security scan is not independent healthcare accreditation. Clinical rehearsal must use fictional information in the separate demonstration or an explicitly approved test environment, never real patient information in the production platform.
Non-clinical account registration
Account setup saves the selected account type, consent and, for professional or facility applicants, a public organisation or practice name. It records a participation status and any administrator decision, date and reviewer reference. The applicant and app administrators can read the application; only administrators can update its review status. Participation approval does not verify a clinician, authorise an organisation or give anyone access to other patients. Sign-in and sign-up use the platform’s secure account experience. Account preferences remain separate from any health profile.
Existing record fields and future clinical workflows
Sign-in uses the platform’s authentication service. You can save an optional preferred name, interface language and larger-text preference to your own account. Appointment and screening requests save your account reference, service choice, preferred date, period, consent and administrative status. They remain private to their owner and app administrators and are not confirmed bookings. Care updates store the patient account reference, category, title, date, details, sharing status and staff attribution. Staff must verify their authority, the recipient, consent and clinical source before sharing. The platform does not independently verify clinical credentials or certify the service as clinically approved. A general enquiry saves the non-clinical details explicitly shown on its form for administrators to review.
Your daily health diary
The clinical diary is currently paused for new entries and edits. Its existing fields cover a dated wellbeing status, symptoms, blood pressure, temperature, pain score and description, food and drink notes, and prescribed medicine or supplement information. Measurements are optional and patient-entered, not clinically verified. Consent is required before saving. New entries and editing remain paused. Owners retain deletion rights over existing entries, and app administrators can read and remove them. Saving does not request clinical review, send an alert or arrange care. Private entries require an online connection and are not stored in the offline education library. Public example check-ins are separate, remain only in memory, and clear on refresh. The health-worker demonstration reads only fictional check-ins from the same visit.
Account-safety questions and map privacy
The optional account questionnaire saves your ownership declaration, whether you recognised an existing clinic reference or had no linked record, a personal/shared-device choice, consent, question version and completion time. Responses are accessible only to their creator and administrators; you can update or withdraw them from the questionnaire page. No password, security answer, identity document, date of birth or clinic identifier is copied into this acknowledgement. It is not proof of identity and never changes access rights. Example responses remain in memory and reset on refresh. The Lagos directory uses public reference listings, not confirmed partner data. The interactive map loads only when requested and sends ordinary network and tile requests to OpenStreetMap; it does not request device geolocation or send account answers or health records. External source and location links open third-party websites.
AI and language
Scripted replies do not send messages to AI. If you explicitly choose Live AI and consent, only a preset general question is sent to an external AI service, without conversation history. Free-text questions are not accepted in this prototype. No chat record is created by this app; provider retention cannot be guaranteed here. New educational content and medical translations require qualified review.
Shared devices and confidentiality
Your browser history, device administrator or someone with access to your account may reveal activity. There are no automatic parental notifications in the fictional examples, but the prototype cannot promise absolute confidentiality.
Requests and future safeguards
Use the general contact form to request privacy information, correction or deletion without including medical details. A complete retention schedule, incident response process, contractual review, lawful Nigerian operating procedures, clinical governance and independent security review are required before any real healthcare-data operation. No compliance certification is claimed.
